Our office may contact you a few days prior to your appointment to confirm the appointment date and time, pre-register standard information and remind you of the upcoming appointment. When checking-in, it may be necessary to obtain copies of insurance ID card(s) in order to bill your insurance company.

Appointments & Cancellations

You may schedule an appointment with a patient care coordinator.

With the exception of serious emergencies, it is expected that you keep your appointments. If you need to reschedule an appointment, it is necessary that you advise the office at least 24 hours in advance. Your consideration in this matter will allow us to accommodate other patients who could benefit from an appointment.

Emergencies

Our office provides on-call service for emergencies. You must call so that the appropriate provider on-call can be contacted. After office hours, an answering service may take your message and immediately contact a provider with your information.

NOTE: It is not possible to schedule emergency appointments online.
